Microneedling Combined with Topical Minoxidil
Study conclusion
Meta-analysis of 10 randomised controlled trials involving 466 patients found that combining microneedling with topical minoxidil significantly increased total hair count compared to minoxidil alone. A large effect size was found (SMD 1.76). However, no significant improvement in hair diameter was seen with the addition of microneedling.
Strength of evidence
This was a systematic review and meta-analysis of 10 RCTs. The score reflects consistent evidence for the combination. It is not higher because hair diameter did not improve significantly, all studies used microneedling as a combination treatment only, and individual trial sample sizes varied.
Who it applies to
Who was studied
Adults with pattern hair loss. 466 participants across 10 RCTs. Microneedling plus topical minoxidil versus topical minoxidil alone.
Who was NOT studied
Microneedling alone without minoxidil. People using minoxidil in forms other than topical solution.
What to look for when shopping
Microneedling is a clinic-based procedure. This evidence is specifically for microneedling added to topical minoxidil — not for home dermarollers or other forms of minoxidil.
What research cannot help you decide
Whether home dermarollers produce equivalent results to clinic microneedling. What happens to hair diameter with combination therapy — no significant improvement was found.
Key findings
- Adding microneedling to topical minoxidil significantly increased total hair count vs minoxidil alone (large effect size, SMD 1.76)
- All 8 meta-analysis studies showed significant increases in total hair count
- Hair diameter did not improve significantly with the addition of microneedling
- All evidence is for microneedling as an add-on to topical minoxidil — not as a standalone treatment
What they did
Researchers searched for all RCTs comparing microneedling plus topical minoxidil against topical minoxidil alone for pattern hair loss. 10 trials with 466 participants were included. Total hair count and hair diameter were the main outcomes combined statistically.
What they found
| Comparison | Result | Significant? |
|---|---|---|
| Microneedling + minoxidil vs minoxidil alone — total hair count | Significant increase (SMD 1.76 — large effect) | Yes |
| Microneedling + minoxidil vs minoxidil alone — hair diameter | No significant improvement | No |
What this study does not show
- 1.Whether microneedling improves hair diameter when added to minoxidil.
- 2.Whether microneedling alone without minoxidil would help.
- 3.What the optimal needle depth or session frequency is.
Limitations
- 1.All evidence is for combination therapy — microneedling was never tested alone vs placebo
- 2.No significant benefit found for hair diameter
- 3.Individual trial sample sizes varied
- 4.Clinic procedure with variability in technique and needling depth across trials
